The Learner Evaluation is completed by a preceptor and is designed to be most often used on a mobile device. The Learner Evaluation can be customized completely. There are ten major topic areas with two topics allowing for two additional sub-questions each. Utilize this form to:

  1. Setting up Evaluation Items for a role[1],
  2. Changing default items or where/when the questions publish.

Each item on the Learner Evaluation can be customized. The Preceptor is asked to rate the learner on a series of items regarding their effectiveness in their new role. Customize each item: Item Name, Choose where to publish (Daily / Formal / Both), and Description. The Minimum Score, Maximum Score, and Steps on the scale are based on the rubric.

The Rubic tab is the second tab on the form; however, understand the control this tab exerts over all the evaluation items, how they are presented, scored, and compared to others. The user is presented with the formatted rubric, a dropdown showing how the array matches the scored (keys) to the display values, and an example item.

There are two text fields that save the instructions that appear at the top of the evaluations:

Evaluation Instrucitons: Enter Text [From left to right consider how much assistance was needed to achieve the goal for each patient.]

This tab controls both the description/instructions on how to score an item, but also the scale for the item and the labels. 

  • The Rubric field is highlighted in yellow and is written in HTML[2].
  • The Array field is highlighted blue and is written so that the website can decipher the list into keys (result) and the value (displayed score).
    • All of the unique keys (matching the integer value saved in the database) are separated by a comma “,” with a space between each.
    • The values are in the same order and also separated by a comma “,” with a space between each.
    • The keys and values are separated by a semi-colon “;”[2].
  • The Minimum Score field, highlighted in orange, is applied to every single item on the Learner Evaluation. This is the lowest value on the scale[3].
  • The Step Between Score field, highlighted in green, designates how sensitive the scale can be; this value is applied to every single item on the Learner Evaluation[3].
  • The Maximum Score field, highlighted in grey, designates the maximum end of the scale and is also applied to every single item on the Learner Evaluation[3].
  • The Minimum Label, highlighted in purple, is a description for the low end of the scale and is written in HTML.
  • The Maximum Label, highlighted in red, is a description for the high end of the scale and is written in HTML.
  • The Learner Rubric is used on Daily/Formal Evaluations, and it is also used on Provider Evaluations when scoring items about the Learner. The Preceptor Evaluation Rubric is set up in the same way.

Recommendation: Click the button to apply the rubric adjustments to the items before editing.

There are ten items scored on a Learner Evaluation that can be customized; items four and five have two additional sub-questions. Each item applies the rubric to a question. 

The evaluation is designed to ask questions that the layperson can understand and score to provide a comprehensive picture of the learner's clinical judgement.

The multiple-choice or open-ended items that appear at the top of the evaluation are created on a record related to the individual learner and are updated through the Learner Update form.

There are shift description dropdown fields that allow the Preceptor to report on their assignment for the shift.

The Goal Templates are designed to facilitate the completion of the Formal Evaluation by having options that the user can select as a goal for the learner. 

Click the button to enter the Edit Screen for the desired goal.

Each topic has the title brought over from the Evaluation Items screen and is set up to have as many as five goal templates.

The Title of the Goal is highlighted in yellow in the figure above.

The Goal that will be displayed and entered on the Formal Evaluation is highlighted in blue. The use of five underscores (_____) will be substituted for the Learner's Name. The goal can be refined from the template before completing the Formal Evaluation by the Orientation Nurse User.

Click on when satisfied with the changes[2] and use the to see an example of how the evaluation will appear to users.


Footnotes

[1] If a total overhaul of Evaluation Items becomes necessary, then it would be best to start using a new Configuration record and start building a new benchmark.

[2] Best Practice: Evaluation Rubrics and items should not be routinely changed. This will corrupt the validity of the benchmark groups as they will no longer be answering the same questions/topics. 

[3] The same rubric and scoring are applied to every question on the evaluation.

[4] The data is visible from the time of submission to the Orientation Nurse Users when logged in. Comments are never visible to Leadership, Learner, or Preceptor Users unless highlighted as a Preceptor Comment. The published data is displayed in summaries such as the Dashboard, Leader Review, and Learner Public Page. It is a best practice to review the data before publishing so the Orientation Nurse can take action, supporting progress, and intervening when encountering challenges.
